Notice of Hiring – Associate Professor IV (College of Medicine)

Qualifications:

• Graduate of Doctor of Medicine
• Holder of Master’s Degree in Health-Related Discipline
• With teaching experience in a medical school for at least three (3) years
• Attended 16 hours of relevant training
• Must be a member of good standing in the accredited professional, specialty or academic society

Requirements for Application:

1. Application Letter indicating the position and the office assignment.
2. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) and attached Work Experience Sheet which can be downloaded at www.csc.gov.ph
3. Copy of Transcript of Records
4. Copy of Diploma
5. Copy of PRC License and Board Rating
6. Copy of certificate of employment from previous employer
7. Copy of certificate of trainings/seminars attended; and
8. Certificate/s of Employment
